STUDENTS: Please read the following carefully before you submit your information. Student File Requirements Please complete all the documents on the information page (including Student File Requirements) and submit the application no sooner than 30 days from your target start date at the facility. Student Clearance Release Form Reliant Human Resources (HR) department will process all documents to clear you to enter the facility. A Student Clearance Release form is available, and you need to complete this form once Reliant HR has cleared you to start. This form must accompany you to the facility and be presented to the Director of Rehabilitation (DOR) on your first day. This form must then be returned to Student Programs on the last day of your rotation. Student Supervision Agreement There is a Student Supervision Agreement that you must print off to take into the facility with you on your first day. Your supervision/clinical instructor (CI) will review this form with you when you start. We recommend that you review it beforehand, so you are prepared to ask any questions. The signed Student Supervision Agreement must be submitted on your first day to Student Programs by you facility’s Director of Rehabilitation (DOR). Feedback Survey Request Once you have completed your rotation, you will receive a feedback survey request from Student Programs. Please complete this survey about your experience with Reliant.
